Populate careplan from evaluation

By allowing us to check a box on the left hand side "add care plan" when completing the evaluation, it would save us from having to go in and out of documentations and would flow nicely, especially when having to send to MD's for signature. The last clinic I worked at used Fusion and they had this feature.
